Grayhawk Family Chiropractic offers a convenient online secure electronic health record system for our patients.  This allows our patients to have complete access to their health record whenever they want!  Your new patient forms can be completed online in the convenience of your own home or office and this information will be sent to us directly so you don't need to worry about bringing any paperwork with you to your first visit!

  • When you call our office to schedule your first appointment, you will be given a PIN number that will enable you to set up your online profile.
  • Click on the link below to log into the Patient Portal
  • Once you are in the Patient Portal, follow the directions to set up your online account - all you will need for this is an email address
  • Once you have activated your account - fill out the 2 required sets of forms Health History Information & Case Intake and you are ready to go!
